BIRTH CERTIFICATES
 
  • Certified copies of Birth Certificates can be obtained for anyone born in the State of Ohio at our local office.
  • Certified copies of Birth Certificates are required for most official uses of vital records, such as school enrollment, driving permit/license, and marriage license.
  • Certified copies of Birth Certificates are printed on security paper and carry the seal of the State of Ohio Registrar.
  • Certified copies of Birth Certificates are $25.00 each.
  • We accept cash and credit or debit cards when paying at the counter. When paying online, you must pay with a credit or debit card. Additional processing fees apply for all credit or debit card transactions, both at the counter and through an online order. The certificate can be picked up in person, or be mailed to you (shipping fee will apply).
 
Ways to request a birth certificate:
Online: The quickest way to apply/receive your certificate is to Apply Online . Please click the online ordering button above and follow the prompts to place an order. We will process these orders within 2 business days. You must pay with a credit or debit card when ordering online (an additional fee applies). The certificate can then be mailed to you or you can choose to pick it up in person.
 
In-person: We are located at 68501 Bannock Rd St. Clairsville Ohio 43950. We ask that you fill the request out online before you come to the office. If you come in for a walk-in request, you will still need to access to our online system to process the request either via our tablet or your cell phone and we can accept cash and credit or debit cards at the counter. An additional fee applies to all card transactions. Once the request is placed, it can take up to 15 minutes to complete your order. The Vital Statistics department will stop printing certificates at 4:00 PM.
 
For the person receiving the certificate, you are REQUIRED to provide:
  • Name (First, Middle, Last)
  • Birthday (MM/DD/YYYY)
  • City of Birth
  • Mother's Maiden Name (First, Middle Last)
 
As an Open Records State, the person applying for the certificate (If not the individual above) will provide:
  • Name (First, Middle, Last)
  • Address
  • Phone Number

DEATH CERTIFICATES
 
  • Certified Copies of Death Certificates are only available for those who have passed away IN Belmont County, Ohio. (Death Certificates are held in the county the event occurs, not resides)
  • Certified copies of Death Certificates are required for most official uses of vital records.

Important Notice regarding Death Certificates: ORC 3705.25(5) Requires that the social security number be redacted off of any death certificate for a person who has died within the last five years unless the person applying for the certificate can prove that they are either a, spouse or legal partner, natural or adopted child, grandchild, or great-grandchild, funeral home, veteran's administration officer, law enforcement agency, executor or administrator of estate, etc.  For a full list of relationship and what qualities as acceptable proof please call (740-695-1202)

  • County birth records from 1908 to present. County death records from 1916 to present, but there are very few from the years 1908-1920.
  • Bellaire City birth and death records from 1908-2000.
  • Martins Ferry City birth records from 1908-2000. Martins Ferry City death records from 1908-present.
 
 
 
 
 
 
GENEALOGY RESEARCH
 
Searches are conducted as time allows. We accept requests through by mail and will return information requested as soon as time allows. No copies of documents are issued. If you require a legal document, a State-certified certificate will be issued at a cost of $25.00. Please note that processing of current documents will take precedence over research of genealogy documents.
